Membership Application Form

Memberships dues are a vital support to our teachers. With them the Home and School Association funds teacher grants to guarantee access to educational materials that otherwise the school will not be able to afford.

We encourage families to send the form and cash or check to the school, this is a way for us to use every single penny of your donation. However, we understand is not always possible, so we had put together the online form for you to use. There are two important parts to follow. First, please fill the form for the school for records. (We use google checkout to ensure a safe payment.), second,  fill the credit card information.

    (You can send it with your child to his/her advisor or to Masterman School Atn: Membership, 1699 Spring Garden Street, Philadelphia, PA 19130)
  • Online Donation (First step: Complete the form for school records, after you have submitted, it will take you to the payment option. For online donations (only) there is a minimum of $11, to cover cost of transaction fees.)
