REQUIRED MEDICAL FORMS AND DOCUMENTATION

The required documentation is as follows:
6th graders: PHYSICAL EXAMINATION FORM 7th graders: DENTAL FORM 11th graders: PHYSICAL EXAMINATION FORM ALL students are required to have immunizations according to the SDP: https://www.philasd.org/studenthealth/#reqim No form is required for this. Parents/guardians are asked to obtain a copy of their child's immunization record from their healthcare provider and send to school in hard copy with the student OR by email to our school nurse, Eloisa Bagley, at [email protected]. Nurse Bagley is happy to answer any further questions! |
